element: implement hint flags

Straightforward approach to implementing suggested hint flags.

Obviously one can up with all sorts of if'ing examples/applications of demuxers
skipping some index or metadata parsing steps depending on configured hint
flags, but not that much point until API to do so is agreed upon and
stabilizes.
