refactor uri fetching

The only modification I have made from the source is to remove the dependency
on GstFragment and use GstAdapter instead. I am not happy about how the
canceling is managed, but I think this is the least intrusive way to patch the
code in. Further optimisation & integration can be dealt with in new patches.
