codecparsers: h264: parse MVC syntax elements

- handle stereoscopic profiles ;
- re-arrange gst_h264_parse_nalu_header() to accomodate SVC ;
- remove slice_parse_ref_pic_list_mvc_modification(). The final ISO
specification fixed the typos ;
- add constraint_set{4,5}_flag
