Not sure, assertion failure started to happen only with the patch applied.

Yes, pipeline changes state like this: NULL -> PLAYING -> NULL -> PLAYING ->
assertion failure.

The sequence above should do the trick. I will try to make a test program to
trigger this.

Dunno about the rest, I hope someone more competent will take a closer look. :)
