mpegtspacketizer: rework TS packet sync and extraction

I've found some issues in the TS parsing code while playing with the
breakmydata element. The sync scan code in mpegtspacketizer is pretty
confusing, and can easily be blocked in an infinite loop when sync is lost.

I've rewritten this code to make it much clearer IMHO, and I've tested it with
against a few TS files with breakmydata.
