update stream segment base time and eos stream if segment end is reached

OK. One more patch. there was another issue were qtdemux was not checking the
end of a strem segment it was only checking the global qtdemux segment.

So, when performing non-flushing seeks with a segment with a stop value below
the qtdemux segment stop, it was not working.
