In answer to your questions:

Yes, it always happens at the start of any such song.

It happens every time a song with higher sampling/bitrates than 16/44 is
played.

Yes, the wav plays fine in VLC, for example.

I should also point out that I have a non-standard audio setup. I think most
people are using junky onboard audio. I have the M-Audio Transit. It sits at
the end of a 12-ft USB cable. Unfortunately I am not confident that the Linux
driver support is entirely up to par compared to basic onboard audio.
