[Bug 747616] flvdemux: ignores audio and video channels that are added too late

This happens because flvdemux emits no-more-pads after a while and from that
point onwards ignores any newly found streams. Instead it should probably do a
"group switch" or similar, or probably can do something more clever with
decodebin3.
