rtpmanager: Reduce the number of upstream GstRTPRetransmissionRequest events.

Rather than sending one GstRTPRetransmissionRequest upstream event for each
sequence number from a RR, send one per FCI entry.  Should reduce upstream
events significantly in high packet loss situations.
