[Bug 684790] isomp4/qtdemux: Whenever a moov atom is found, restart the demuxer

Basic idea is, duplication/existence/new stream check will not be done in
_parse_trak() function.

Then, whenever got new moov, call _expose_streams() and decides
reuse/expose/remove stream in that function.
