You could have told us that there was a critical assertion. Having to dig that
out from your log it a pain:

** (ko:31351): CRITICAL **: gst_v4l2_buffer_pool_process: assertion
'gst_buffer_pool_is_active (bpool)' failed

That need to be fixed, after an assertion don't expect anything to go well.
After this assert the device is not properly closed, hence the next open() not
giving back the same FD number.
