flvmux: Resend stream headers on caps change

This adds some rudimentary support for resending the stream headers when the
incoming caps change. In particular, this fixes sending H.264 with changing
resolution to NGINX-RTMP, which does not update its internal state correctly
when encountering just inline SPS/PPS NALUs.
