i'm going to proposal a radical core API change.

i think the caps nego callback (connect) should be per-element instead
of per-pad.

Justification:  Look at stereo2mono.  Both pads are sent to the
same callback anyway.  When writing the code, folks need to think
in the mode that any pad could be connected (or not) at any time
in any order.  Making the callback per-element would encourage
the correct way of thinking about caps nego.  QED
