mp4 file chapterisation

I'm currently muxing a video source (h264) and a audio source (aac) into a single mp4 file with mp4mux element (somekind of recording type feature) . I would like to add chapters (tracks) info into the mix so at a later time when playing a long one hour mp4 file you could skip to different location in the player.  First of all can this be done with in a single MP4 file or do I have to create multiple mp4 files? Any suggestions would be greatly helpful.
