Ok, next thing to do is: instead of fakesink sync=true use ximagesink or
xvimagesink or whatever, to test whether A/V sync is fine if you use
those.

If that also fails, then try with just video-sink=ximagesink  or
whatever (or just leave video-sink and audio-sink unset, that should
make it use auto{video,audio}sink).
