No, FAULT_MODE is not always set. Before this change,
it was always set for !SCRATCH_PAGE cases. Before this
change scratch page and fault mode are mutually exclusive.
With this change, we can have scratch under fault mode, so

So bullet 2 and 3 are already in the test.
I looked how to add bullet 1. Adding it to the existing xe_exec function would
make the codes very very hard to read and maintain. So I decided to add
a separate function to test scratch page. This patch will be dropped.
