Correction:

Under troubleshooting, step 2 should read:

2) Moved the left monitor to the right side and connected it to the HDMI cable
and moved the right monitor to the left side and connected it to the DVI cable.
The problem **DID NOT** follow the monitor which indicates the problem is
somewhere between the DVI port and the Intel driver.
