Can you please try what happens when you manually add the mode:

$ xrandr --newmode "3840x2160_60.00"  712.34  3840 4152 4576 5312  2160 2161
2164 2235  -HSync +Vsync
$ xrandr --addmode HDMI2 "3840x2160_60.00"
$ xrandr --output HDMI2 --mode "3840x2160_60.00"

You need to test this with the patch v3 otherwise the kernel will reject the
mode when you try to use it.
