GPU error state

This may be a duplicate, not sure.

I have a Lenovo T520 that's docked with an external monitor connected via
Displayport. Also have an additional screen connected via a plugable USB
display adapter (assuming that's not related).

No idea how to reproduce, but the frequency seems to be increasing of late.

System environment:
