In the dmesg I can also see

[    8.223153] i915: switched off

Looking at the 3.17.6 kernel it seems we won't re-enable the PCI device
afterwards in response to a switcheroo ON command. Could you try v3.19-rc2 or
git://anongit.freedesktop.org/drm-intel ? There is a related i915 switcheroo
fix in those that should address this issue.
