It happens multiple times a day on almost every machine at work. All machines
has intel built in graphics and the only common thing about this bug is that
there was always web browser running some page with flash.

We use Optiplex 380, 390, 3010 and 7010 with Ubuntu. Those problems started
when we switched from Ubuntu 10.04 to 12.04 and even now using HWE kernel does
not solve the problem.
