dmesg log file with debugging enabled

I have an Asus z170 gaming-itx/ac motherboard which has one dp and two hdmi
connections. I have three Samsung S27D850 monitors.

When I connect only one monitor (dp or hdmi) everything is fine.
When I connect two monitors (dp/hdmi or hdmi/hdmi) everything is fine.

However, when I connect all three monitors, neither of the monitors connected
to the hdmi ports will work. Only the dp monitor works.

When I open the display program in ubuntu with three monitors connected, it
shows me all three (correctly identified as Samsung monitors). 1 and 2 are
enabled (even although 2 is blank) and 3 is disabled.

I try to enable monitor 3 but it won't accept it.

I've tried both hotplugging the 3rd monitor and having them all connected at
startup.

I've contacted Asus about this, and they told me that when they tested this
with linux they had the same symptoms, but when they tested with Windows all 3
monitors were working.

Seems to me therefore that the problem lies with the linux driver and not with
my hardware.

I'm using Intel Graphics Installer 1.4.0 (with latest updates downloaded) and
the latest kernel (4.5 rc5)
