Just an update from me:
I have not seen this problem for a while now. 

I've been using the same HDMI adaptor since I reported this problem. I think
around 2017-03 the problem was fixed, at least on my system.

I tested it with various devices. From 40" Flat TV to 24" Dell Full HD
monitors.

Right now running a 4k Samsung display without issues.

Works on Dell XPS 9350 with I6200U - Intel® HD Graphics 520
 and 9360 with i7-8560u - Intel® UHD Graphics 620

Using 4.10.0 kernel.
