The write into the HWSP went astray. This can happen due to a GPU failure or
invalid PTE/TLB for the HWSP. How reproducible is the problem, and can you
please try drm-tip [https://cgit.freedesktop.org/drm-tip]?

Hmm, something else to consider is that there were no CS interrupts either.

What happens if you completely disable iommu in your kconfig?
