Notes from debug: 

Issue appears to occur anytime the display switches from a 74.xxx MHz mode
(23.98, 24, 29.97, 30 Hz) to a higher clock frequency (148 Mhz and 296 Mhz at
50, 59.94, and 60 Hz).

Long HPD pulse restores output at the selected resolution.

PLL programming during failure is correct based on dpll_hw_state in the logs
during success and failure.

CDCLK remains constant at 316MHz.

i915_power_domain_info shows identical "Use counts" during working and
non-working cases.

Reviewed GLK workaround database for similar issue. Nothing Found.
