I believe the root cause of the majority of these occurrences is radio
frequency interference.

This particular Dell motherboard design seems to be susceptible to RFI, and
research suggests that the Optiplex 5040 motherboard design was later revised
by Dell to address this issue.

Installing RF chokes (ferrite cores) on all cables (e.g. USB, power,
displayport) has reduced the frequency of these issues at least tenfold.
