Isn't this a solved problem?

I think the problem of starting with no output detected had been fixed 
in the Xserver
code. I think you need 1.5. I'm not running that version yet to confirm 
it works
but looking at the changes in the server it seems right now.

It is possible to patch the intel driver to work around the issue if you 
are stuck
on an older Xserver.

Of course you have to setup Xorg.conf to tell the server that it is o.k. 
to ignore the
presence of a monitor.
