[LGM-org] Reimbursement form - quick fix needed

Hi,

I logged in the 2016 website to edit the form for reimbursements.

Surprise! The form does not exist.

It’s a bad surprise, if this is confirmed.

I am not sure I have been consulted with that change and in any event I am
not sure I have received a warning telling me this was going to happen so I
could address the issue — or try to avoid it.

Last year, same story, I had to redo the form because the decision was made
to change the form plugin. No warning. I was left in total ignorance of the
disappearing of my main tool to fulfill our engagement of making this
process as smooth and efficient as possible.

This year, I am not going to redo this form again! However we need this
form. It’s not an option, it’s compulsory.

To track down all the info that is needed to process reimbursements, we
need a reliable tool and we use those forms for years now. There is no way
out. And I will certainly not listen to anyone telling me how to handle
this otherwise, or that we don’t need a form. I am terribly disappointed.

If a good soul could find a way to import the form from last year, it would
be awesome. We seem to have kept the same plugin "Formidable". There maybe
a way to address this.

I logged in last year’s website and hurray, the form still exits. The only
concern now is to copy this form over to the new site and to edit it for
2016.

Please note that whoever takes the challenge, the form has a couple
conditional fields. So, needless to mention that it takes some observations
skills to recreate this if the conditions don’t follow, for any reason. And
we need to test. I will be happy to help with that last part and take the
responsibility of putting the form online once it appears in the forms
list. Just let me know when done.
