[LGM] url rewrite on lgm

hi

thanks to tobias and michael's hints and help, we now have some sort of
url rewrite on the lgm server:

- there is a directory for each navigation entry with a tiny php
  script that redirects to the "index?pageid=..." page
- for this reason, if you want to add a page to the navigation, you now
  have to manually a link to the custom navigation.
- the links in the banner have still to be modified
