Sample docx with issues as provided by client

Attached docx document has issues on opening (missing formatting etc.).
Attached PDF  in following note is how it looks after imported to MS Office 97
and PDF'd with PDF Creator (not by me!).

4 others docs with similar issues were provided by the same client. Not sure
why a French public body would be providing docx...
