I now upload 3 file.
Step 3 can produce by 
Tool -> MailMerge -> Save or Print

P.S. 
mailmerge will create additional useless blank page with each entry.
so if I have one entry in database(and my mailmerge template just consum one
page), it will create one more invisible blank page.
