Problem description: 
Spell Check fails to detect correction if the corrected word appears more than
once in the dialog box.

Steps to reproduce:
1. Spell check dialog highlights unrecognised word.
2. Add word to dictionary
3. Dialog highlights next error or unrecognised word in same text - the word
had just been added to the dictionary, or changed (either manually or by
selecting from list of suggestions)

Current behavior:
As in steps

Expected behavior:
Spell check should (perhaps) refresh the dialog after the change made, and
recognise the second word. See screenshot.
