I finally got the 2nd type of corruption again, and it does seem only related
to headings.

Steps:
1. converted old .docx file in MS Office 2007 to .odt (via MSO 2007)
2. opened up blank notes file that had some imported formatting
3. opened up converted .odt and copy contents into blank document with correct
formatting
4. view corruption -- it happened several times, even with closing and
reopening LO completely. However, this time, the PDF that I generated was
correct. The "e" handing off the page in one of the attempts at opening the
file actually scrolled with the page rather than being attached to it's initial
position.

I'll attached the saved file of the corruption from two different attempts that
both showed corruption.
