I also note that the toolbar does not appear in 6.1 when creating a Label with
database fields. The toolbar does appear in the 6.2 build mentioned above. I do
not know if this is a different symptom of the same bug, or should be raised as
a different bug.

Reproduction:
1. New > Labels.
2. Select an appropriate "Database", a "Table" and "Database Field" from the
right-hand drop down menus.
3. Click the left arrow (tooltip says "Insert") to insert a field.
4. Click "New Document".

If you save that new document and re-open it, the toolbar does appear.
