First line of a list of endnotes showing the corruption

I have a problem with the layout of endnotes when converting to Word. I am able
to work around it most of the time, but when publishing to Smashwords they
require the use of Word 1997-2003. When converting to this format, the first
endnote is corrupted with an indent inserted and the endnote pushed onto a
separate line (this worsens with each save), and, importantly, the page
settings for all the endnotes are changed back to the default.

This means that the page sizes are different for the endnotes when compared to
the rest of the document, which is not acceptable.
