I reproduce in 6.2+ like this in Windows with virtual printer:
1. open Calc, add some data and set Format-Page-Paper Format as Letter 
2. open File-Printer Settings-(Printer) Properties and set page as A4
3. go to File-Print and print

Actual Results: I see letter paper size.
Expected Results: we should see A4 paper size.

I didn't understand why you have steps: save, exit, reopen, print again.
Also, I don't see a purpose of this bug with real paper printer, because it
will adjust Letter to A4 and print. 
Can you please explain what exactly you expect.
