What is still incorrect:

- The frame on the first page is shown as a black square.

What is (still) fixed:

- The index doesn't have space between the number and the titles.
- In the PDF, paragraph 2.10 is now properly shown on page 2.

I'm currently on Ubuntu 18.04 with LibreOffice Version: 

Version: 6.1.1.2
Build ID: libreoffice-6.1.1.2-snap1
CPU threads: 4; OS: Linux 4.15; UI render: default; VCL: gtk3; 
Locale: en-US (en_US.UTF-8); Calc: group threaded
