I have no idea, how the wrong layer state comes into the document. Current
versions should work correctly for new documents. Documents once repaired
should stay OK too. It might be, that saving the document was interrupted. Such
wrong layer state was reported on the Ask site too, put up to now no scenario
is known, that produces the wrong layer state, so a "fix" is not possible.
