... and as I wrote in comment #5 I'm seeing this issue.

I don't know if this is relevant, but:

On mouseover of the first (i.e. correctly positioned) icon, the tooltip shows
"LibreOffice Writer" and over the far right (second icon after LO is lauched),
the tooltip shows "soffice.bin"
