Example file with image outside header

I'm seeing the same issue, but the image is outside the header. It behaves
exactly as described by the reporter - it obscures the background colour of the
table, but not the borders, and the 'send to back' command has no effect.
(Interestingly, 'bring to front' also has no effect).
