1.  With the attachment 155876:  Only Heading 1 was shown the PDF viewer. 
(which also seems to be the case with the initial report.

2.  Modified the Style for Table of Figures and Bibliography -- changing the
Outline Level from Text Body to Level 1.  Now all three headers are shown with
the PDF export.  Will attached the revised .odt file

If that was what was desired, then maybe this is notabug?
