When creating multiple grouped tick boxes within a form template under docx

form appears to work correctly throughout. However, when the file is saved,
closed and relaunched, all of the tick boxes disappear. There is a gap where
the boxes should be.
Steps to Reproduce:

2.Create a group tick box
3. Reorganise tick boxes, font and alignments in design mode
4. exit design mode to check operation and tick exclusivity functions ok
5. Copy/paste grouped tick boxes into desired locations, renumbering box
identifier to prevent box inter-dependency.
6. Test all boxes can tick correctly and retention of choice.
7. Print to check box control function does not prevent tick choice output.
8. Save file
