Description:
The test is easy to design: create a new writer document, insert chapters with
subchapters, export it as epub and then read it in any epub reader. bye.

Steps to Reproduce:
1.create a new writer document, 
2.insert chapters with subchapters, 
3.export it as epub 
4.read it in any epub reader

Actual Results:
the table of contents has just Chapters title (i.e. Title 1 style), no
sub-chapters, sub-sub-chapters (Title 2, Title 3...)

Expected Results:
to generate a table of contents using all titles not only Title 1
