QR Code is integrated in LO from 6.4.0.
For example on Writer, you can generate one from:
- Insert menu
- Object
- QR Code...
Could you give it a try?

About the extension itself, what's the name of it? You could uninstall the
extension, rename your LO directory profile (see
https://wiki.documentfoundation.org/UserProfile#GNU.2FLinux) and give a new
try.
