Sorry the bring this thing up again.. As this dialog is being redesigned
recently..

And yes, I didn't follow the whole discussion back then.. so pointers or
summary :-). Not that it really matters.. the current design isn't it. Someone
needs to shuffle some things around with a dialog builder (or make a mock-up).

And no, I don't think people will be overblown by a few more settings visible 

Take the size - of the Insert table of content..
