Cannot repro with "Protect against manual changes" which is default on ToC.
Without "Protect..", repro in 71+, crash or no responding on update ToC.
But steps make no sense, because paste is in ToC so the whole document becomes
index.
Are unrealistic steps a bug? Let Xisco decide.
