I have set bug 132745 as a DUP of this bug report, as they both report a crash
when attempting to connect to the macOS Address Book, albeit via different call
mechanisms :

- this bug report - access via the Create DB wizard
- bug report 132745 - access via the Exchange Address datasource wizard
