and your steps (caption for a frame), but not problem to insert Fig.

So no idea, why it doesn't work for you.

If you like, you can reset your Libreoffice profile
(https://wiki.documentfoundation.org/UserProfile) and re-test, to be certain
the reported issue is not related to corruption in the user profile.
