On pc Debian x86-64 with LO Debian package 6.4.3 + gtk3, I don't reproduce
this.
I just gave a try with your file + by default options (I selected all fields,
then next and finish).

Could you give a look at https://wiki.documentfoundation.org/QA/FirstSteps ?
Also, did you try by default options when creating form?
