Description:
In the attache document, headings are not listed in Cross-reference table. The
source of the glitch is unknown. A document from year 2019 was recycled and
rewritten. In the process, the bug has been discovered.

Steps to Reproduce:
Not reproducible by me.

Actual Results:
Headings are not listed in the Cross-reference table

Expected Results:
Headings shall be listed in the Cross-reference table


Reproducible: Always
